Faculty Web Account Request Form

CCRI provides faculty web accounts as a vehicle for faculty to distribute course-related documentation and contact information for their students. To request a faculty web account for space to create a web page, please use the form below. Please note that the turn-around time for account creation is three working days. When your account is created you will receive an e-mail message and instructions on accessing the web server to build your pages.

Please Note: you will need to activate a CCRI domain account in order for your web site to be created. If you do not have a domain account, (Visiting Lecturers/Adjunct Faculty do not automatically get a domain acct.), kindly print, fill-out and have your department head sign and return to the IT dept., ATTN: Helpdesk, the Campus Domain Account Application Form. Please do this before submitting the electronic form below.

Please refer to online documentation about using FrontPage to create your web site. For more information please call the Help Desk at 825-1112.

Once your page has been created, please send an e-mail to Beth at edowling@ccri.edu with your URL (http://faculty.ccri.edu/yourusername) and ask her to add your name to the Faculty Web Index.

Please Note: You must have a domain account before requesting a faculty web site.
